Insurance
I am not in-network with any insurance companies. I am not a Medicare provider. My assistant will provide you with a superbill/receipt that you can submit to your insurance company for out-of-network coverage. You need to contact your insurance company to determine whether or not you have out-of-network benefits and the kind of out-of-network coverage that you have.
Payments
Patient credit cards are stored in a secure system and are charged within a week after the visit. When your card is charged, you will be sent a receipt/superbill through the Patient Portal. I accept Mastercard, Visa, and checks.
Cancellation Policy
I ask that my patients provide two business days (48 hours) notice if they cannot attend an appointment. I charge my full fee for late cancellations and no-shows. Patients who late cancel or no-show their first appointment will need to pay the missed appointment fee before being allowed to reschedule.
Refills
Please request refills at least 48 hours in advance by contacting my assistant, either by phone or through the Patient Portal. There is a fee for refills needed urgently outside of business hours.
Patients are encouraged to use the Patient Portal for any cancellations, refill requests, or non-urgent matters. The Patient Portal maintains an accurate list of your current appointments.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
